New research awards from the National Institutes of Health aim to help patients with multiple chronic conditions.

Estimates are that 3 in 4 adults over age 65 suffer from two or more chronic medical conditions—such as diabetes mellitus, chronic kidney disease, hypertension, and chronic pain. As patients develop more chronic conditions, they are likely to use more health care services and suffer from:

  • unnecessary hospitalizations
  • adverse drug reactions
  • declining functional status

One funded study, for example, will investigate advanced care planning in nursing homes, where patients and staff address difficult decisions about resuscitation, use of ventilators in the last stages of illness, and similar important care issues.
